Steps before trying Erection Drugs.


Help boost effectiveness. For men who respond poorly to erection drugs, it may help to combine the medication with the over-the-counter supplement, ArginMax. Researchers at University of California, Davis, worked with men with ED, who did not get much benefit from Viagra. The men took Viagra plus either ArginMax or a placebo. After four weeks, erections improved significantly in 22 percent of those taking the placebo, but among men using ArginMax, the figure was 60 percent.

Know the erection drug’s intended use. Viagra was originally approved for medically diagnosed, persistent ED. The original ads featured elderly Senator Bob Dole talking about how it helped his medical problem.

But once a drug is approved for any reason, doctors are free to prescribe it for other, so-called “off label” uses. Today, all erection medications are most widely used as “erection insurance” for men in their 40s to 60s who have balky erections and don’t want to worry about them. Steps before trying Viagra



Know that, despite their success, erection drugs also have limitations. They are ineffective in about 25 percent of cases. As severity of ED increases, their effectiveness decreases. For example, the drugs work well in many men with the beginnings of diabetic ED, but less well in diabetics with considerable cardiovascular and neurological damage. In men with mild erection balkiness, these drugs may not work in some situations—if a man feels particularly stressed, distracted or alienated from the sexual experience.

Be aware that if you take any nitrate drug such as nitrate medication for heart disease, nitroglycerine for angina, or the party drug, amyl nitrate (“poppers”), you should not use erection drugs such as Viagra, **** or ****. Be prepared to take more over time. The latest studies suggest that some men need to increase their dose over time. University of Alabama researchers tracked 150 men who took Viagra regularly for two years or more. During that period one-third increased their dose from 50 to 100 mg.

DRUG CLASS AND MECHANISM: Tadalafil is an oral drug that is used for treating impotence (the inability to attain or maintain a penile erection). It is in a class of drugs called phosphodiesterase inhibitors that also includes sildenafil (Viagra) and vardenafil (Levitra).
Erection of the penis is caused by the filling of the penis with blood. Filling occurs because the blood vessels that bring blood to the penis increase in size and deliver more blood to the penis, and, at the same time, the blood vessels that take blood away from the penis decrease in size and remove less blood from the penis.
Sexual stimulation that leads to an erection causes the production and release of nitric oxide in the penis. The nitric oxide causes an enzyme, guanylate cyclase, to produce cyclic guanosine monophosphate (cGMP). It is the cGMP that is primarily responsible for increasing and decreasing the size of the blood vessels carrying blood to and from the penis, respectively, and causing the erection. When the cGMP is destroyed by another enzyme, phosphodiesterase-5, the blood vessels return to their normal size, blood leaves the penis, and the erection ends. Tadalafil prevents phosphodiesterase-5 from destroying cGMP so that cGMP stays around longer. The persistence of cGMP leads to a more prolonged engorgement of the penis with blood.

DOSING: For most individuals, the recommended starting dose of tadalafil is 10 mg per day taken before sexual activity. Depending on the adequacy of the response or side effects, the dose may be increased to 20 mg or decreased to 5 mg a day. The effect of tadalafil may last up to 36 hours. Individuals who are taking medications that increase the blood levels of tadalafil should not exceed a total dose of 10 mg in 72 hours (See drug interactions). Tadalafil may be taken with or without food since food does not affect its absorption from the intestine.
DRUG INTERACTIONS: The breakdown and elimination of tadalafil from the body may be decreased by erythromycin, ketoconazole (Nizoral), itraconazole (Sporanox), indinavir (Crixivan) and ritonavir (Norvir). Therefore, these drugs may increase the levels of tadalafil in the blood. If these drugs are being used at the same time as tadalafil, the dose of tadalafil should be reduced in order to avoid side effects from high levels of tadalafil.
Tadalafil exaggerates the increases in heart rate and lowering of blood pressure caused by nitrates, e.g. nitroglycerin, isosorbide dinitrate (Isordil), isosorbide mononitrate (Imdur, Ismo, Monoket), nitroglycerin (Nitro-Dur, Transderm-Nitro) that are used primarily for treating heart pain (angina). In patients who take nitrates for angina, tadalafil could cause heart pain or possibly even a heart attack by exaggerating the increase in heart rate and the lowering of blood pressure. Therefore, tadalafil should not be used with nitrates. Tadalafil also exaggerates the blood pressure lowering effects of some alpha-blocking drugs e.g., terazosin (Hytrin) that primarily are used for treating high blood pressure or enlargement of the prostate. Individuals who take these alpha-blockers should be on a stable dose of the alpha-blocker before tadalafil is started. In such situations, tadalafil should be started at the lowest dose. If the patient is already taking tadalafil, the alpha-blocker should be started at the lowest dose. Tadalafil and alcohol both lower blood pressure. Therefore, combining tadalafil with alcohol may cause excessive drops in blood pressure and cause dizziness, headaches and increased heart rate.
PREGNANCY: Tadalafil is not approved for use in women.
NURSING MOTHERS: Tadalafil has not been evaluated in women who are breastfeeding.
SIDE EFFECTS: The most common side effects of tadalafil are facial flushing (reddening), headaches, stomach upset, diarrhea, flu-like symptoms and nausea. Tadalafil also may cause low blood pressure, blurred vision and changes in color vision, and abnormal ejaculation. Tadalafil has been associated with prolonged erections or priapism (painful erections lasting more than 6 hours). Patients should seek immediate medical help if they experience an erection lasting more than 4 hours. Use of tadalafil, most often in patients with pre-existing heart disease, has been associated with chest pain, heart attacks, death, strokes, palpitations and increased heart rate; however, it is not clear whether these side effects are due to tadalafil, concurrent use of nitrates, or the sexual activity itself. Rare cases of sudden loss of hearing have been reported with phosphodiesterase inhibitors such as tadalafil, sometimes associated with ringing in the ears and dizziness. If changes in hearing occur, patients should stop their tadalafil and seek immediate medical attention.

DRUG CLASS AND MECHANISM: It has been estimated that impotence affects 140 million men worldwide. Over half of all men with impotence are thought to have some physical (medical) cause. The remainder are believed to have psychogenic causes of impotence. Medical causes of impotence include diabetes and circulatory, neurological or urological conditions.
Penile erection is caused by the engorgement of the penis with blood. This engorgement occurs when the blood vessels delivering blood to the penis increase the delivery of blood and the blood vessels carrying blood away from the penis decrease the removal of blood. Under normal conditions, sexual stimulation leads to the production and release of nitric oxide in the penis. Nitric oxide then activates the enzyme, guanylate cyclase, which causes the production of cyclic guanosine monophosphate (cGMP). It is the cGMP that is primarily responsible for the erection by affecting the amount of blood that the blood vessels deliver and remove from the penis.
Sildenafil also reduces the pressure in the pulmonary artery in a serious condition called pulmonary arterial hypertension.

When sildenafil is taken with a high fat meal, the rate of absorption is reduced, with an average delay in the time to maximal concentration of 1 hour.
DRUG INTERACTIONS: Sildenafil increases the effects of the blood pressure lowering medications. It also increases the blood pressure lowering effects of nitrates, e.g. isosorbide dinitrate (Isordil), isosorbide mononitrate (Imdur, Ismo, Monoket), nitroglycerin (Nitro-Dur, Transderm-Nitro) that are used primarily for treating angina. Patients taking nitrates should not receive sildenafil.
Cimetidine (Tagamet), erythromycin, ketoconazole (Nizoral), itraconazole (Sporanox) and mibefradil (Posicor) can cause marked increases in the amount of sildenafil in the body. Patients taking these medications should be observed carefully if sildenafil is used.
It is expected that rifampin will decrease blood levels of sildenafil and probably reduce its effectiveness.
PREGNANCY: Although extensive testing in animals has demonstrated no negative effects on the fetus, sildenafil has not been studied in pregnant women. There is no effect on sperm count or motility of sperm in men.
NURSING MOTHERS: It is not known whether sildenafil is excreted into breast milk.
SIDE EFFECTS: Approximately 15% of persons taking sildenafil experience side effects. The most common side effects are facial flushing (1 in 10), headaches (1 in 6), stomach pain, nasal congestion, nausea, diarrhea, and an inability to differentiate between the colors green and blue. Rare cases of sudden loss of hearing have been reported with phosphodiesterase inhibitors such as sildenafil, sometimes with ringing in the ears and dizziness. If changes in hearing occur, patients should discontinue their sildenafil and seek immediate medical attention.

Men suffering from certain heart diseases and ED are given a second chance

Medicine has stepped into a superior era of evolution after one of its most recently developed clinical trials, as results reveal it. It appears that, finally, after many years of controversy and waiting, men suffering from erectile dysfunction and certain heart conditions not only are allowed to take ED drugs, but these may even improve the quality of their lives.
The study supporting this statement was published in a reputed medical release, The Journal of Sexual Medicine, and conducted by Doctor Graham Jackson in England.
Doctor Jackson is the initiator of a sexual counseling program for men with cardiac problems and erectile dysfunction in his private clinic in England, the first and only medical clinic supporting a program with these particular profile and destination. Doctor Jackson’s scientific study was conducted on 425 male patients with stable coronary disease and erectile dysfunction. The challenge of the trial was to testify that ceasing the oral nitrate therapy commonly prescribed for these patients and replacing it with a beta-blockade or a calcium antagonist therapy may allow the safe use of oral ED drugs in improving the erectile function in these patients. Surprisingly, in more than 85% of the patients included in the study, discontinuing the therapy with nitrates and the administration of Viagra, Cialis or Levitra lead to significant erectile improvement. What counts even more is that none of the patients dealt with adverse cardiac reactions.
The importance of this study is pointed out by the Journal’s Editor-in Chief himself, Irwin Goldstein, MD, stating that this is a first in proving that stable coronary disease patients are allowed to safely use PDE-5 Inhibitors, like Viagra, Cialis or Levitra, and improve the erectile function, as long as they cease the treatment with oral nitrates and opt for a beta-blockade or calcium antagonist therapy. Secondly, on a larger view, we can get a clear picture of how multi-dimensional medicine is and how relevant professional exchange between cardiologists and urologists turned out for this study.
The incidence of angina pectoris in the United States reaches 17,5‰, while 8% of the American adults suffer from one or another form of heart condition. That is approximately 22 million individuals in need for treatment which may include nitrates, beta-blockers or calcium antagonist.

Doctors say a growing number of women are taking the anti-impotence pharmaceutical drug Viagra despite the lack of studies that prove it will amplify Libido feelings for female sexual drive, desires and responses.
"I've heard reports from all around the country of doctors prescribing it off-label to women," Dr. John Brand, director of the Kelsey Institute for Research in Sex Gender and Reproduction.

Three have reported better feelings of sexual drives, desires and arousal responses.

Muscular dystrophy is a genetic condition causing wasting of the muscles.
The first signs of muscular weakness appear at roughly age five, leading to a progressive loss in the ability to walk by the age of 13.
People with the condition are also at a higher risk of heart failure due to a weakening of the muscles which keep the organ pumping strongly.
For this reason, many people with Duchenne muscular dystrophy - the most common form of the condition - die in early life, often in their 20s or 30s.

The researchers also inserted a gene that increased cGMP production in the mice's heart cells, and found that this helped the animals to maintain normal cardiac function.
Dr Marita Pohlschmidt, director of research at the Muscular Dystrophy Campaign, said the research was interesting.
However, she added: "It is important to remember that benefits seen in animals do not always translate into human medicine.

My girlfriend has a high sexual appetite

I am a 41 year old man who, thus far, never had the opportunity to have sexual intercourse, and this now presents itself with a lady I am very fond of and am getting very serious about. BACKGROUND: Tadalafil is a phosphodiesterase 5 (PDE5) inhibitor approved in >30 countries for the treatment of erectile dysfunction (ED). It has been shown to improve erectile function compared with placebo in Phase III studies, but clinical experience comparing tadalafil with the PDE5 inhibitor sildenafil citrate is lacking.
OBJECTIVE: This study compared patient preference for tadalafil 20 mg or sildenafil 50 mg during initial treatment for ED. It also compared the tolerability of the 2 agents at these doses.
METHODS: This randomized, double-blind, fixed-dose, 2-period crossover trial took place at 13 sites in the United States and Germany. Patients were randomized 1:1 to receive 4 weeks of treatment with tadalafil 20 mg or sildenafil 50 mg, followed by the alternative treatment, to be taken as needed up to once daily before sexual activity.
RESULTS: The study enrolled 215 men with ED, 109 randomized to the tadalafil-sildenafil sequence and 106 to the sildenafil-tadalafil sequence. Their mean age was 49.8 years; 84.7% were sildenafil naive and 15.3% had undergone a previous inadequate trial of sildenafil. Most patients had moderate ED (60.5%) of >or=1 year's duration (74.9%). Of 190 evaluable patients, 126 (66.3%) preferred to initiate treatment with tadalafil, compared with 64 (33.7%) with sildenafil exposure. Both medications were well tolerated, with no significant differences in the incidence of treatment-emergent adverse events. Headache (11.2% tadalafil, 8.8% sildenafil), dyspepsia [indigestion](6.0% and 4.2%, respectively), nasopharyngitis [inflammation of the nose and pharynx](4.7% and 2.8%), and flushing (2.8% and 4.7%) were the most common adverse events.
CONCLUSIONS: Tadalafil 20 mg was preferred to sildenafil 50 mg for the initiation of ED therapy in this study population. Both medications were well tolerated.

The Search for the Female Equivalent of Viagra

Even in the most sexually liberated and self-satisfied of nations, many people still yearn to burn more, to feel ready for bedding no matter what the clock says and to desire their partner of 23 years as much as they did when their love was brand new.
The market is saturated with books on how to revive a flagging libido or spice up monotonous sex, and sex therapists say “lack of desire” is one of the most common complaints they hear from patients, particularly women.
And though there may be legitimate sociological or personal underpinnings to that diminished desire — chronic overwork and stress, a hostile workplace, a slovenly or unsupportive spouse — still the age-old search continues for a simple chemical fix, Cupid encapsulated, a thrill in a pill.
Since the spectacular success of Viagra and similar drugs, the pharmaceutical industry has been searching for the female equivalent of Viagra — a treatment that would do for women’s most common sexual complaint, lack of desire, what sildenafil did for men’s, erectile dysfunction.
Initial trials of Viagra in women proved highly disappointing. True, the drug enhanced engorgement of vaginal tissue, just as it had of the penis, but that extra bit of pelvic swelling did nothing to amplify women’s desire for or enjoyment of sex.
What is needed for the treatment of so-called female hypoactive sexual desire disorder, researchers decided, is a reasonably safe and effective drug that acts on the central nervous system, on the pleasure centers of the brain or the sensory circuitry that serves them.
For a while, many sex therapists and doctors were optimistic about Procter & Gamble’s Intrinsa, a testosterone patch that delivers small transdermal pulses of the sex hormone thought to play a crucial if poorly understood role in male and female libido alike. But in 2005, the Food and Drug Administration refused to approve Intrinsa, declaring that its medical risks outweighed whatever modest and spotty benefits it might offer.
More recently, another potentially promising treatment for hypoactive desire has been making its way through clinical trials. The compound, called bremelanotide, is a synthetic version of a hormone involved in skin pigmentation, and it was initially developed by Palatin Technologies of New Jersey as a potential tanning agent to help prevent skin cancer. But when male college students participating in early safety tests began reporting that the drug sometimes gave them erections, the company began exploring bremelanotide’s utility as a treatment for sexual disorders.
Studies in rodents demonstrated that the drug not only gave male rats spontaneous erections, but also fomented sexual excitement in female rats, prompting them to wiggle their ears, hop excitedly, rub noses with males and otherwise display unmistakable hallmarks of rodent arousal.
Importantly, the females responded to the drug only under laboratory conditions where they could maintain a sense of control over the mating game. Take away the female’s opportunity to escape or proceed at her preferred pace, and no amount of bremelanotide would get those ears to wiggle. In other words, Annette M. Shadiack, director of biological research of Palatin, said, “this doesn’t look like a potential date-rape drug.”
Inspired by the rodent work, the company decided to give the drug a whirl on women. Results from a pilot study of 26 postmenopausal women with diagnoses of sexual arousal disorder suggest that bremelanotide may well have some mild aphrodisiacal properties.
Responding to questionnaires after taking either the drug or a dummy pill, 73 percent of the women on bremelanotide reported feeling genitally aroused, compared with 23 percent given the placebo; and 43 percent of the bremelanotide group said the treatment augmented their sexual desire, against only 19 percent of those on dummy pills.
Women in the treatment group also were slightly more likely to have sex with their partners during the course of the trial than were those in the control group, although who initiated the romps was not specified.
Larger trials of the drug at some 20 clinical centers around the United States are now under way. Among other things, the researchers will try adjusting the dosage to see if more bremelanotide may provoke a more robust response with a minimum of unpleasant or embarrassing side effects.
For example, researchers are as yet unsure whether sustained use of bremelanotide will end up doing what the drug was meant to do in the first place, and bestow on its beaming clients a truly healthy tan.

"The recreational use of Viagra by apparently healthy women and men is now occurring in Britain," Judith Aldridge and Fiona Measham, senior researchers at the University of Manchester, said in a letter posted on The British Medical Journal Internet site. Aldridge and Measham interviewed young adults at nightclubs in northern England within weeks of the launch of the drug in Britain in 1998. Customers and staff said the triangular blue pills made by the U.S. pharmaceutical company Pfizer were readily available for 16 pounds (US$24) for a 50 mg tablet. Most of the men and women who admitted taking Viagra used it simultaneously with other drugs and alcohol.
"Of particular concern is the combination use of Viagra with amyl nitrite ('poppers'), both of which dilate blood vessels, and can result in a dangerous drop in blood pressure potentially causing heart attack or stroke," the researchers said.
All of the people who admitted using Viagra reported positive effects: enhanced sexual desire and lovemaking, and feelings of warmth. They got the drug from friends, partners, dealers, sex shops, or via the Internet.
Ironically, the drug is not as freely available for many impotent men who need it. Fearful that demand could bankrupt the country's government-funded National Health Service (NHS), Health Secretary Frank Dobson issued proposals suggesting Viagra should only be prescribed in the NHS system for men whose impotence is caused by a physical condition.
The move infuriated doctors who said the policy was unacceptable and unethical. The British Medical Association advised doctors to defy the government and prescribe the drug to patients who would benefit from it.

Before taking Viagra


Do not take Viagra if you are also using a nitrate drug for chest pain or heart problems. This includes nitroglycerin (Nitrostat, Nitrolingual, Nitro-Dur, Nitro-Bid, and others), isosorbide dinitrate (Dilatrate-SR, Isordil, Sorbitrate), and isosorbide mononitrate (Imdur, ISMO, Monoket). Nitrates are also found in some recreational drugs such as amyl nitrate or nitrite ("poppers"). Taking Viagra with a nitrate medicine can cause a serious decrease in blood pressure, leading to fainting, stroke, or heart attack. Before taking Viagra, tell your doctor if you are allergic to any drugs, or if you have:
- heart disease or heart rhythm problems;
- a recent history (in the past 6 months) of a heart attack, stroke, or heart rhythm disorder;
- congestive heart failure;
- high or low blood pressure;
- coronary artery disease;
- liver disease;
- kidney disease;
- a blood cell disorder such as sickle cell anemia, multiple myeloma, or leukemia;
- a bleeding disorder such as hemophilia;
- a stomach ulcer;
- retinitis pigmentosa (an inherited condition of the eye);
- a physical deformity of the penis (such as Peyronie's disease); or

If you have any of these conditions, you may need a dose adjustment or special tests to safely take Viagra.

What's Viagra?


Sildenafil citrate, sold as Viagra, Revatio and under various other trade names, is a drug used to treat erectile dysfunction and pulmonary arterial hypertension (PAH).
